Dirty RSE Laundry -  Management and Communication Fails

We are teaching a class at RWTH on management and communication in RSE as part of piloting the RSE-Master curriculum (shared project deRSE and GI). For this we are searching for people from RSE-practice that could contribute a one-page case study of a difficult situation where the students could apply their theoretical knowledge could you contribute something like this?
